Enthält: Die Ratsherren Diderick van Grolle und Jasper Merkanck, Provisoren der Armen der Liebfrauen- und Antoniuskapelle vor dem Mauritztor, geben bekannt, daß sie und Hinrich Krone, Amtmann der genannten Kapelle, von Domvikar Gerd Wysseman, genannt Trippemeker, eine Rentverschreibung des Arnd Belholt über jährlich 2 Rheinische Gulden auf eine Hauptsumme von 40 Gulden und eine Rentverschreibung des Alhard Pladis über jährlich 9 Scheffel Roggen zugunsten der genannten Armen der genannten Kapelle erhalten haben. Die Urkunden soll der Amtmann verwahren. Von der Rente soll er dem Pastor jeden Tag in der Fastenzeit und jeden Samstag in der Zeit zwischen Ostern und Pfingsten 4 Pfennige und den 4 Priestern je 2 Pfennige geben, dafür daß sie an den genannten Tagen nachmittags zwischen fünf und sechs Uhr zusammenkommen, um die Kollekten zu lesen und in der Fastenzeit die Marienantifon Salve Regina und zwischen Ostern und Pfingsten Regina Coeli zu singen. Der Amtmann soll des weiteren für Glocken und Kerzen sorgen und die Jungen verköstigen, die die Tropen und Avemaria singen, wofür er jährlich 2 Schillinge erhält. Den Rest der Rente soll er den Armen aushändigen. Falls die Rente abgelöst wird, soll sie neu angelegt werden. Zeugen: Die Vikare Ludike Wysseman und Johannes Hoinse. Angehängtes Siegel des Diderick van Grolle.
